Aging upper eyelid surgery

We perform upper eyelid surgery in an outpatient surgery center. IV sedation is given to make patient comfortable, but awake enough so that the patient can open and close eyes when requested. This is only way to make sure that the shape and size is symmetrical.

We believe that there are several aspect of aging upper eyelid surgery that is essential.

  • Most patient who complain about droopy eyelid skin, wishes for all of the loose skin to be taken out. However, too much excision of skin can lead to very unnatural look and even complications such as inability to close eyes completely. We believe in conservative approach to skin excision. Remove enough skin to rejuvenate the appearance, but maintain natural soft look to the upper lid contour.
  • Normal process of aging involves loss of fat in the orbital space. This can lead to gaunt and tired look. Also, if severe enough, can seem to be another supratarsal fold. For these patients, we believe in strongly that part of the blepharoplasty procedure should be to fill the orbital space with either fat or alloderm to achieve the full youthful appearance.
  • From the upper blepharoplasty incisions, several ancillary procedures can be peformed if necessary. For people with deep corrugator lines, the corrugator muscle can be weakened. For slight ptosis of the eyebrow, the eyebrow position can also be elevated from the upper blepharoplasty.
  • Clean and rejuvenated outer end of the supratarsal fold is important. This is where the skin excision should be performed the most.

This is an operation that can freshen up the eyes as well as an aging face. In upper eyelid surgery, the surgeon first marks the individual lines and creases of the lids in order to keep the scars as invisible as possible along these natural folds. The incision is made, and excess fat, muscle, and loose skin are removed conservatively to avoid hollowing. The skin incision is closed using a combination of interrupted and continuous non-dissolving sutures. The incision is usually hidden well since there is enough amountof skin that will overlap the scar.
